The driver of a 2002 Honda Accord who died earlier this month in a crash that may have involved a ruptured Takata airbag inflator hadn’t been notified that his vehicle was recalled in June 2014 for the suspect inflators, a Honda spokesman said.

Authorities have yet to determine the official cause of death in the Jan. 18 accident , and Honda said it hasn’t yet inspected the vehicle to confirm whether a rupture occurred. If a ruptured inflator is confirmed as the cause, it would mark the sixth death linked to inflators made by Takata.
